Mounjaro for Menopause: Can It Help With Weight Gain & HRT?

The menopause can be tough – managing weight gain, dealing with hormone fluctuations, and other physical changes can all take their toll. The good news is that there are medications that can help make the process less difficult. 

Mounjaro (active ingredient tirzepatide) is a new-generation weight loss drug that has become popular for those struggling with obesity or type 2 diabetes.

But what about Mounjaro for menopause? Can it help manage symptoms or weight gain during this stage of life – and is it safe to take alongside hormone replacement therapy (HRT)?

What is The Menopause?

Menopause is the stage of a woman’s life that marks the end of her reproductive years – when menstrual cycles end.

Most women go through this stage between the ages of 45 and 55, triggered by a steep decline in oestrogen and progesterone levels (however, there are, of course, exceptions to this). 

Some common symptoms of menopause include: 

  • Hot flushes
  • Mood swings
  • Sleep disturbances
  • Vaginal dryness
  • Weight gain (especially around the abdomen)

But why do women experience weight gain during menopause?

The hormonal shifts associated with menopause can significantly affect metabolism, which makes weight gain more likely – even without changes to diet or exercise. That’s why many women seek support for managing menopause-related weight changes.

What is Mounjaro?

Mounjaro is the brand name for tirzepatide, a dual GIP and GLP-1 receptor agonist. It works by mimicking two gut hormones that help to regulate appetite and blood sugar levels. 

These combined effects lead to:

  • Increased feelings of fullness
  • Reduced food intake
  • Better blood glucose control

Originally approved for type 2 diabetes, Mounjaro is now widely used to promote weight loss – and clinical research suggests it may be especially effective in women. 

On average, female participants lost around 24% of their body weight, compared to approximately 18% in male participants.

Can You Take Mounjaro During Menopause?

Yes, many women take Mounjaro during perimenopause or postmenopause as it can help manage the weight gained as a result of hormonal changes.

If you are already experiencing hormonal changes that affect digestion or appetite, these side effects may feel more intense. Hormonal Changes

Although rare, and not an officially recognised side effect, some women report spotting and bleeding whilst taking GLP-1 medications – particularly women who have gone through menopause. 

This can be distressing, especially if you haven’t had a period in a long time. The exact cause isn’t fully understood, but some potential explanations include: 

  • Changes in oral HRT absorption – Mounjaro may alter how your body processes other medications, including oral HRT, which could lead to hormonal imbalances that trigger spotting.
  • Rapid weight loss – A sudden reduction in body fat can impact oestrogen levels, as fat tissue plays a role in hormone production. This may temporarily disrupt your hormone balance and menstrual cycle.
  • Hormonal fluctuations – Even though Mounjaro doesn’t contain “female” hormones, it may indirectly influence your endocrine system or interact with medications that do.

Unsuitable for Pregnancy

Mounjaro is not recommended for use during pregnancy, if you're planning to conceive, or if you are currently breastfeeding. If you’re perimenopausal and still have the potential to become pregnant, it’s important to use reliable non-oral contraception while taking Mounjaro. 

Tirzepatide may harm an unborn baby, and its effects during pregnancy have not been fully studied.

How Mounjaro Affects HRT

Currently, there’s no evidence to suggest Mounjaro interferes with the effectiveness of hormone replacement therapy (HRT).

However, Mounjaro slows down gastric emptying. This may affect how your body absorbs oral medications, including oral hormone replacement therapy (HRT). When HRT isn’t absorbed properly, it may lead to unbalanced hormone levels,  potentially leading to breakthrough bleeding.

HRT and Mounjaro can also both impact metabolism, fluid retention, and appetite, so it’s important to monitor how your body responds when combining the two. Some women on HRT may be more sensitive to side effects like nausea or headaches when they begin Mounjaro – so be sure to liaise with a medical professional (such as ourselves here at Dr Frank’s Weight Loss Clinic) when taking the two medications. 

Women also appear to lose more weight on GLP1 medications when taking HRT – In a 2024 study, women using weight loss medications alongside HRT lost 16% of their body weight after one year. Women who were not on HRT lost just 12%. 

The British Menopause Society suggests reviewing oral HRT regimens carefully in patients using Mounjaro. This may involve switching to HRT patches or gels, which will not be affected by Mounjaro’s action in the gut.

How to Manage Menopause Weight Gain

Weight gain during menopause is common, with more than half of women experiencing weight gain during menopause. 

Weight gain during menopause is often caused by:

  • Declining oestrogen levels
  • Reduced muscle mass
  • Slower metabolism
  • Changes in insulin sensitivity

The good news is that although weight gain is common, it doesn’t have to be inevitable. Here are some ways you can prevent, manage or reduce weight gain during menopause: 

  • Balanced diet – Reducing processed foods, increasing protein intake, and balancing macronutrients can help stabilise blood sugar and support a healthy weight
  • Regular exercise– Building muscle can counteract the metabolic slowdown and support fat loss, and activities like walking, swimming, or cycling can help burn calories and improve heart health.
  • Stress management – Chronic stress raises cortisol levels, which can lead to abdominal weight gain. Practices like yoga, meditation, and good sleep hygiene can help.

Can Mounjaro Help With Menopause Weight Gain?

Yes, Mounjaro can be extremely helpful in managing weight gain during and after menopause. It can help to counteract the metabolic slowdown and increased fat storage that many women experience during menopause.

A UK clinical trial in 2023 found that women using Mounjaro alongside tailored lifestyle changes lost 12–15% of their body weight over 72 weeks, with an average waist circumference reduction of 8 cm. Participants also experienced an 18% improvement in glucose regulation after six months.

Patients can lose up to 26.6% of their body weight through Mounjaro. This averages around 60lbs in two years. 

Although Mounjaro can be very effective in managing weight gain and promoting weight loss, there is currently no research showing that Mounjaro directly alters menopause symptoms like hot flashes or mood changes.
