Does Mounjaro Cause Bleeding After Menopause?

Mounjaro (tirzepatide) is a popular weight loss and type 2 diabetes medication, and has recently gained attention for its potential benefits in managing menopausal symptoms – specifically weight gain. 

Close to 70% of women reported gaining weight during menopause, with weight gain continuing post menopause. 

Mounjaro can help – but as more women start taking it, more people are wondering whether Mounjaro can cause bleeding during or after menopause.

Postmenopausal bleeding is not a listed side effect in clinical trials, but some users do report a potential link, especially in those using HRT. But what's really going on? Let's find out.

Can You Take HRT With Mounjaro? 

Yes, many women take HRT and Mounjaro together without any issues. However, you should make sure you monitor how your body responds, especially during the first few weeks of treatment.

Mounjaro mimics the GLP-1 and GIP hormones, slowing down digestion, regulating blood sugar, and reducing appetite. This, however, can also affect how your body absorbs oral medications such as HRT. 

If you are taking HRT, this could lead to fluctuations in hormone levels, potentially leading to spotting or unexpected bleeding. 

If you’re concerned, speak to your GP or menopause specialist or one of the Dr Frank’s team. They may suggest that you adjust your HRT dose or switch to a transdermal patch instead. 

How Does Mounjaro Affect HRT?

Mounjaro delays gastric emptying, so it can affect how your body absorbs oral medications – including HRT (Hormone replacement therapy). 

If your HRT doesn't get absorbed properly, then your hormone levels may become unbalanced. If you still have a uterus, this can cause breakthrough bleeding.

Should I Switch to an HRT Patch If I'm Taking Mounjaro?

If you’re using oral HRT and have noticed bleeding or changes in your symptoms since starting Mounjaro, it may be worth discussing a switch to transdermal HRT (patches, gels, or sprays) with your doctor. These options completely bypass your digestive system, so are less likely to be affected by Mounjaro.

Can Mounjaro Cause Vaginal Bleeding? 

In short, yes – Mounjaro may indirectly cause vaginal spotting in patients taking HRT. Although this is not mentioned in clinical trials or studies, some patients have reported this as a side effect of Mounjaro. 

Some potential causes of this include:

  • Changes in HRT absorption
  • Rapid weight loss affecting oestrogen levels
  • Hormonal fluctuations due to medication interactions
  • Underlying conditions such as endometrial hyperplasia (unrelated to Mounjaro)

You should look into all unexpected vaginal bleeding, regardless of whether you are taking Mounjaro or not. Always speak with your healthcare provider if you experience vaginal bleeding, to rule out anything serious. 

Other Side Effects of Mounjaro 

Like any medication, Mounjaro can cause side effects. The most common include:

  • Nausea
  • Headaches
  • Constipation or diarrhoea
  • Fatigue
  • Appetite changes

Mounjaro & HRT: BMS Advice

There is currently no formal evidence currently suggesting a dangerous interaction between Mounjaro and HRT. However, the BMS (British Menopause Society) does recommend that if you are postmenopausal, you should monitor any symptoms closely. 

If you are taking Mounjaro, you should take oral HRT with caution – or have your routine reviewed by a specialist.

It’s best to avoid oral HRT and opt for a non-oral option instead, as these are generally not affected by Mounjaro.

Oral Progesterone Consider switching to the Mirena coil or increasing the oral progesterone dose for 4 weeks after starting or increasing Mounjaro.

When Should I See a Doctor? 

If you experience any vaginal bleeding after menopause, you should contact your doctor. See your doctor if:

  • You’ve been postmenopausal for over 12 months and have experienced any bleeding
  • Bleeding is heavy or persistent
  • You’ve recently started a new medication (like Mounjaro or HRT)
  • You have a personal or family history of uterine conditions

Your GP may recommend a pelvic ultrasound, blood tests, or a referral to a gynaecologist to rule out more serious causes.
